1.6 Million children at risk in Afghanistan winter – Malnutrition

Around 1.6 million children of under 5 yrs age and 625000 child bearing age women are at risk of
dying this winter due to malnutrition exacerbated by drought and high prices of food.
Kabul, Nov 25, 2008: Dr. SM Amin Fatimie, Minister of Public Health of Afghanistan participated
in a meeting of National Committee on Disaster Management and Response on Sunday 23 of
November 2008. The meeting was chaired by His Excellency, Abdul Karim Khalili, second vice

president here in Kabul. The participants were cabinet ministers, ambassadors of friendly
countries in Kabul, representatives of UN agencies and donor communities and relevant
governmental and non-governmental organizations. Mr. Khalili once again handed over an official
appeal to the donor and UN representatives to support Afghan government in accessing 129
districts prone to disaster in 22 provinces in the coming winter and to address food shortages and
health problems threatening 6.5 million population who are currently living below the poverty line
in Afghanistan. Out of this 6.5 million general population, 1.6 million are children under 5 yrs age
and 625000 are women of childbearing age, the most vulnerable age groups. Provision and
maintaining of health care services for these vulnerable groups need around $2.5 million,
according to the rough calculation of MoPH.
The plight has been exacerbated by the drought, high prices of food and loss of livestock due to
continuing drought in 2008. We fear that a humanitarian crisis will be imminent and villagers in
those districts might lose a big number of their livestock in the coming winter as well.
“Poverty is one of the most important social determinants of health and nutrition and a major
cause of under nutrition and ill health in Afghanistan, especially in those 129 districts. It also
contributes to the spread of diseases and undermining efficiency of health care services in
Afghanistan and will help unleash population growth. We once again convey agonies and cries of
poor and miserable people of Afghanistan and reiterate that the destiny of world has kinked to the
destiny of Afghanistan” said Dr. Fatimie at the meeting. He also added that: “If international
community does not respond properly and timely to the appeal, the progress gained so far in
different sectors e.g. mortality reduction among children and women will lose its momentum or
even reverse.”
Food security is very important factor for the reduction of morbidity and mortality of many
diseases. For example, case fatality due to tuberculosis has dropped from 23000 in 2002 to 7800
in 2007. This was possible because of successful TB control programme and reasonable food
security in Afghanistan, especially at rural settings. We forecast a very high morbidity and
mortality due to the diseases which are already under control if the current threat is not properly
addressed now, before arrival of the harsh period of winter in January and February 2009.
Ministry of Public Health is impartially and without any form of discrimination delivering health
services to the needy people in all corners of Afghanistan and request all parties to respect this
policy and support health care providers to fulfill this noble job.

Guatemala: Hazard Identification Tool – Floods, 03 Nov 2008

Objective
The objective of the Hazard Identification Tool (HIT) is
to alert the UN Country Team and emergency responders to
potential secondary risks after a natural disaster posed
by large infrastructure and industrial facilities
containing hazardous materials located in the affected
area. This information can be shared with competent local
and national authorities as appropriate. Any actual
secondary risk should be addressed at the earliest
possible stage.

Methodology

The HIT provides the user with the (expected) location of
hazards in the affected area. In addition, the substances
that are expected to be present in these facilities are
listed, as also the hazard type for the whole of the
substances. The last column gives the estimated impact
type of the hazard. The methodology of the HIT is based on
the Flash Environmental Assessment Tool (FEAT), a rapid
assessment methodology to detect the most acute hazards to
human health and the environment after natural disasters.

Event

Continuous rains have caused serious flooding in
Guatemala. Most affected are the departamentos of Alta
Verapaz, Izabal, Peten and Quiche, with Peten being 100%
flooded. Due to instability of slopes, mud- and landslides
have occurred.

The Joint UNEP/OCHA Environment Unit

The Joint UNEP/OCHA Environment Unit is the United Nations
mechanism to mobilize and coordinate the international
response to environmental emergencies caused by natural
disaster, technological accidents and complex emergencies.

On this day in 1991, the so-called “perfect storm” hits the North Atlantic producing remarkably large waves along the New England and Canadian coasts. Over the next several days, the storm spread its fury over the ocean off the coast of Canada. The fishing boat Andrea Gail and its six-member crew were lost in the storm. The disaster spawned the best-selling book The Perfect Storm by Sebastian Junger and a blockbuster Hollywood movie of the same name.

 

On October 27, Hurricane Grace formed near Bermuda and moved north toward the coast of the southeastern United States. Two days later, Grace continued to move north, where it encountered a massive low pressure system moving south from Canada. The clash of systems over the Atlantic Ocean caused 40-to-80-foot waves on October 30–unconfirmed reports put the waves at more than 100 feet in some locations. This massive surf caused extensive coastal flooding, particularly in Massachusetts; damage was also sustained as far south as Jamaica and as far north as Newfoundland.

 

The storm continued to churn in the Atlantic on October 31; it was nicknamed the “Halloween storm.” It came ashore on November 2 along the Nova Scotia coast, then, as it moved northeast over the Gulf Stream waters, it made a highly unusual transition into a hurricane. The National Hurricane Center made the decision not to name the storm for fear it would alarm and confuse local residents. It was only the eighth hurricane not given a name since the naming of hurricanes began in 1950.

 

Meanwhile, as the storm developed, the crew of the 70-foot fishing boat Andrea Gail was fishing for swordfish in the Grand Banks of the North Atlantic. The Andrea Gail was last heard from on October 28. When the boat did not return to port on November 1 as scheduled, rescue teams were sent out.

 

The week-long search for the Andrea Gail and a possible cause of its demise were documented in Junger’s book, which became a national bestseller. Neither the Andrea Gail nor its crew– David Sullivan and Robert Shatford of Gloucester, Mass.; William Tyne, Dale Murphy and Michael Moran of Bradenton Beach, Fla.; and Alfred Pierre of New York City– was ever found. 

Mobile Field Hospital to be developed into New England Disaster Training Center and model for civilian mobile hospital response

U.S. Congressman John B. Larson (CT-01) and Connecticut Department of Public Health (DPH) Commissioner Dr. J. Robert Galvin today announced $8 million in federal funding to enhance the capacity of Connecticut’s mobile field hospital and develop it into a national resource for training and building emergency response systems. 

 

The Department of Defense funds will be used to build a Center of Excellence in disaster response training, the centerpiece of which is the Ottilie W. Lundgren Memorial Field Hospital, a state-of-the-art 100-bed mobile field hospital.  The hospital assembles in hours and can be ready to triage and treat hundreds of patients during any public health emergency.  It was named in 2006 in honor of a Connecticut woman who died from inhalational anthrax in 2001. 

 

“This funding will bring together the best of our military and public health assets to develop a disaster training area and a national model for field-based patient care delivery,” Congressman Larson said during a press conference at Brainard Airport.  “It will showcase to the country the immense skills and resources of Connecticut.”

 

“At any given moment, Connecticut or the nation could be faced with a disaster that results in an overwhelming number of sick or injured people.  The mobile field hospital can alleviate the stresses that natural and man-made disasters put on our statewide urgent care system,” stated Dr. Galvin.  “This mobile hospital is a powerful tool in the public health emergency preparedness arsenal, and these funds will strengthen its effectiveness and scope.” 

 

The funding will support a partnership between DPH, the Connecticut Air Guard, the Connecticut Fire Academy and the state’s 31 acute care hospitals to develop policies and protocols for the 100-bed facility.  Education and training courses will focus on events and scenarios related to bioterrorism, infectious disease outbreaks and mass casualty disaster response requiring deployment of the hospital.

 

To sustain an asset such as this mobile field hospital, it is important to provide multiple uses for it when not deployed during a disaster.  Two such uses include a New England Disaster Training Center and a National Demonstration Model.  The hospital will be developed into a national Center of Excellence for capacity building in emergency response systems, including training civilian, governmental and military organizations from outside of Connecticut that are interested in developing similar programs or would benefit from having access to this facility for education and training. 

 

The Ottilie W. Lundgren Memorial Field Hospital is deployable as a flexible configuration of four 25-bed units that operate jointly or independently to provide triage and treatment anywhere in the state in the event of a mass casualty, or to support an acute care hospital after catastrophic structural or mechanical failure.  During long-term deployments, resources from the state’s 31 acute care facilities will staff the hospital.

Earthquake in Balochistan, Pakistan: OCHA situation report No. 1

HIGHLIGHTS
– An Earthquake of magnitude 6.4 has hit
Balochistan province in south-western
Pakistan leaving more than 80 dead
(government confirmed figure) and dozens
critically injured.

– Numbers are likely to rise as more
information is coming in and assessment
teams are reaching remote areas.

– Immediate needs include food, shelter,
emergency medical assistance and safe
drinking water to survivors in rural
areas.

SITUATION

1. An earthquake of magnitude 6.4 has hit
Balochistan province in south-western
Pakistan on 29 October. According to the
US Geological Survey, the epicenter of the
quake was in Chiltan mountains, 80
kilometers northwest of Quetta. The first
tremor struck at 4:09 am local time (23:09
GMT) at a depth of 10 kilometer while the
second one came at 5:15 am. The affected
region is the mountainous area extending
from Ziarat, about 110 KMs northeast of
Quetta to Pishin, Qilla Abdullah to Chaman
(border town along Afghan border).
Provincial Disaster Management Authority
(PDMA) reports that the worst hit area
falls within villages Khanozai and Topa
Achakzai in eastern Pishin district and
Wachun Kawas village in Ziarat district
and possibly Harnai district (east of
Ziarat).

2. Media and civil organisations are
putting the death toll at 160 whereas the
government figures have confirmed 80
deaths so far. Dozens of individual are
reportedly critically injured. The typical
houses in the area are traditionally made
of mud or mud-bricks with low resistance
against earthquake. Hundreds of houses
reportedly collapsed and electricity and
communication system also failed. Scores
of people have been reported to be trapped
under the rubble while several houses and
buildings destroyed due to the earthquake.
Being a mountainous region, accessibility
to the areas might pose a problem. Some
villages were reportedly affected by
landslides triggered by the earthquake.
WHO reported that the road from Khanozai
to Loralia was blocked due to landslides.
The total number of victims and affected
population is likely to rise as more
information is coming in and assessment
teams are reaching remote areas.

3. There is little information on the
scale of damage in smaller valleys and
regions off the main routes. In addition,
the winter has already started and
temperature reaches 5°C at night.
Immediate needs include food, shelter,
emergency medical assistance and safe
drinking water to survivors in rural
areas.

NATIONAL RESPONSE

4. NDMA is sending its first dispatch of
emergency relief items and mobilising a
search and rescue team. 5,000 tents, 5,000
blankets, 15,000 plastic sheets, winter
clothing, emergency food items have been
dispatched. A 100-bed field hospital is
being deployed in Quetta, possibly in
Ziarat. Another field hospital is on
standby in Rawalpindi and will be deployed
on needs basis. The government has imposed
an emergency in all the hospitals of the
province particularly in Ziarat where
dozens of injured persons are under
treatment. PDMA has further requested for
2,000 tents, 5,000 blankets, food covering
20,000 affected people and 4 New Emergency
Health Kits (NEHKS). One kit is enough for
a population of 10,000 for 3 months.

5. According to Inter-Services Public
Relations (ISPR), Pakistan army
contingents and three C-130 aircraft have
been sent to Wachun, Kawas in Ziarat
district and Toba Achakzai in Pishin
district for rescue operations along with
mobile medical teams. The Government is
also working on clearing the roads.

INTERNATIONAL RESPONSE

6. WHO is sending one New Emergency Health
Kit, two Trauma Kits (part A and B) and
Five Mini Emergency Health Kits which will
reach Quetta by 31 October. WHO team is
also part of the joint UN Assessment
Mission to Ziarat and Pishin districts.

7. UNICEF is working with the government
and other UN agencies to coordinate a
response. It is also part of the joint UN
rapid assessment mission to assess damage
to the worst affected districts. UNICEF is
ready to mobilise its contingency stocks
as part of the relief operation.

8. Oxfam, Save the Children and
MSF-Holland have sent assessment and
response teams today morning to Ziarat and
are ready to intervene in relief activity.

9. District Coordination Office (DCO)
Ziarat has mobilized Ziarat-based NGOs and
presently they are involved in initial
rescue efforts and damage assessment. IUCN
Juniper Project Team is part of the
assessment mission and is presently in
Kawas in Ziarat. According to the initial
reports by IUCN Villages Malik Payo Khan
and Tarrai have been destroyed to the
ground.

10. According to ICRC, two ICRC teams have
reached the affected area and are
assessing the situation and the needs. The
Pakistan Red Crescent Society (PRCS) has
sent two teams to the earthquake-hit
region to distribute relief supplies for
100 families. The team includes 28 staff
and volunteers, as well as two mobile
health teams. The ICRC intends to send
medical teams to the region. A surgical
and medical team will fly from Peshawar
early on Thursday. The IFRC is also
sending a support team to the region,
including a disaster management expert, a
health specialist and an information
officer.

11. The Japanese government decided to
dispatch relief goods consisting of 120
tents and 600 blankets.

12. CARE is shipping 100 winterized
family tents to the affected area near the
provincial capital of Quetta. CARE is also
preparing to ship additional shelter and
emergency supplies and dispatch a medical
team to treat injured survivors.

13. The Government has not yet requested
international assistance.

COORDINATION

14. Two coordination meetings were held
in Quetta respectively chaired by the
Minister PDMA and Tourism and the
Provincial Minister for Inter Provincial
Coordination Department. Two teams
including representatives of UN agencies
were constituted and will go to the worst
affected districts for rapid assessments.

15. An emergency health cluster meeting
will take place in Islamabad on 30
October.

16. OCHA is supporting the RC Office in
Islamabad and a Regional Disaster Response
Advisor from the Regional Office in
Bangkok is now present in Pakistan. OCHA
is also sending an officer to Quetta to
participate in the assessment mission.

New facility to accelerate Australia’s international disaster response

A warehouse containing essential emergency relief supplies opening today will enable Australian humanitarian agencies to respond faster to disasters in the region.

Parliamentary Secretary for International Development Assistance, Bob McMullan, today opened the Joint Emergency Stores Warehouse in Brisbane, a partnership between the Australian Government and three humanitarian relief agencies.

The joint arrangement of the facility will lower administration costs, ensuring more aid is delivered to those who need it. The warehouse will contain about 100 tonnes of supplies, valued at approximately $1 million.

“Australia is the regional leader in providing critical humanitarian and emergency relief and is often the first to respond when a natural disaster or crisis strikes our region,” Mr McMullan said.

“We are now well into cyclone season, and the Asia-Pacific is particularly vulnerable to tropical cyclones, earthquakes, volcanic eruptions, tsunamis, floods and droughts, so it is imperative that we are ready and well equipped to help nearby countries.”

This pioneering facility will be run in partnership with the Australian Agency for International Development (AusAID) and Australian NGOs Oxfam, World Vision and the Australian Red Cross.

Red Cross Chief Executive Officer Robert Tickner said, “As we have seen in the past, the speedy provision of clean water, food and emergency shelter can make the difference in the aftermath of a disaster. This facility is an excellent example of the level of cooperation between the Australian Government and our three agencies, and will allow for much faster, coordinated responses to disasters across the Asia-Pacific region.”

Oxfam Executive Director, Andrew Hewett said, “Having stocks of emergency relief items on hand in Brisbane means that Oxfam can get assistance to those hit hard by disaster in our region much more quickly. Saving hours in an emergency can mean saving lives.”

Chief Executive of World Vision Australia, Tim Costello said, “The 2004 Boxing Day tsunami demonstrated the central role that the Australian Government and NGOs play in disaster relief. Since then we have seen unparalleled cooperation between aid agencies and the Australian Government. This warehouse is the sum of that partnership. The Australian public are rightly always asking about the efficiency of overseas aid efforts. This warehouse will have an enormous impact on our ability to work cooperatively and effectively when disasters strike.”

On average Australia provides assistance to over 30 humanitarian and emergency situations every year. This year, the Government provided assistance to victims of cyclones in Burma and Bangladesh, the Chinese earthquake, floods and landslides in Indonesia and humanitarian food aid to Kenya, Zimbabwe and Afghanistan.

In 2008-09, Australia will provide almost $320 million to support humanitarian, emergency and refugee programs.
